Chatbots for Postpartum Mood Support.

This randomized controlled trial tested the feasibility and early effectiveness of a mental health chatbot — adapted with perinatal-specific content — for supporting mood in postpartum women. A total of 192 English-speaking postpartum women were randomized to receive either usual care alone or usual care plus access to an AI chatbot.

The study explored whether AI-driven, conversational tools could offer accessible and stigma-free mental health support during the sensitive period after childbirth.

What The Study Found

Depression and anxiety symptoms were measured in the postpartum women at baseline, and again at 2, 4, and 6 weeks using validated clinical scales. The results show that a chatbot can be both well-accepted and potentially effective for mood management in the general postpartum population — even among women without clinical depression.

AI Chatbots Can Help Manage Postpartum Mood.

While the effects on anxiety and stress remain limited, the study confirmed AI’s potential to reduce depressive symptoms in young people, particularly those at an early stage of distress.

High satisfaction and acceptability. The chatbot was well received: 91% of users were satisfied or highly satisfied. Most participants (80%) felt comfortable using a mobile app to manage mood and mental health.
Reduction in depression symptoms. The chatbot group showed a greater reduction in depression scores over six weeks compared to the usual care group.
Effect strongest in women with mild symptoms. Among women who began the study with slightly elevated depression scores, the chatbot led to greater improvements — suggesting it may be most helpful for those with mild depressive symptoms.

Emotional connection matters.

The study highlights that many women face stigma when seeking mental health services postpartum. The chatbot offers an accessible and private way to engage with mental health support, potentially facilitating emotional expression and connection where traditional avenues might be difficult to access or feel intimidating.

Therapeutic Relationship. Participants in the chatbot group reported establishing an affective bond with the chatbot.
CBT and IPT Elements. The tested chatbot delivered psychotherapeutic techniques derived from Cognitive Behavioral Therapy (CBT) and Interpersonal Psychotherapy (IPT), which are designed to support mood management and foster a therapeutic interaction.

Why This Matters

The study demonstrates that conversational AI tools can play a meaningful role in supporting postpartum women, a group often underserved or stigmatized within traditional mental health systems. By offering round-the-clock, judgment-free support, mental health chatbots can complement professional care and reach those who might not otherwise seek help.

The tested chatbot integrated psychotherapeutic techniques and perinatal-specific content.
